Josh Cesaro-Moxley (Administrator), Nick Cesaro-Moxley (Environmental Manager), Paulette Vrem (Director of Nursing, Board Certified Gerontological Nurse, MSN, BSN, RN)

Heart to Home was founded in 2007 by three compassionate individuals (Paulette, Nick and Josh).  Their mission was to create a long term care option that would improve peoples lives and build stronger communities.  Through this vision they built some of the first 'green' residential care homes for seniors in the Twin Cities Minnesota metro area.  As a result of these new options in senior care and memory care they have successfully allowed seniors to stay in the community and avoid moving to large institutional nursing facilities.  Each residential care home is just that... home.  In addition to creating a beautiful home environment for seniors to enjoy they have also successfully implemented a model of care that is truly catered to the individual.  To learn more about Heart to Home call (651) 454-5250.

Kayla VeldmanCertified Nursing Assistant and House Manager at Mendota Manor.  Kayla has worked with Heart to Home since 2009.  She has received extensive dementia care training both in-house, through The Grace Geriatric Behavioral Health Unit and by Teepa Snow.  In addition she has received extensive end-of-life / hospice training from nurses and doctors with Saint Croix Hospice.  She has received numerous accolades from families at Heart to Home and is working towards obtaining her Registered Nursing Degree.     

Marlena Godinez, Certified Nursing Assistant and House Manager at Mendakota Manor.  Marlena has worked at Heart to Home since 2012.  Prior to her work at Heart to Home she gained valuable experience in various long-term care settings.  She has received specialized training via Teepa Snow (Dementia Care Expert), hospice care nurses and several programs hosted by Care Providers of Minnesota.  She always takes the initiative to go above and beyond with our residents to ensure that they feel right at home, and loved.

Emily Kinny, Certified Nursing Assistant at Mendakota Manor.  Emily has worked for Heart to Home since 2010.  She has worked in long term care settings since 2006 and has a Bachelor Degree in Secondary Education from UW River Falls.  She has received extensive training in dementia care, volunteers at Sister Kenny Rehab Institute and is working towards her Certified Occupational Therapy Assistant (COTA) degree at St Catherine University.  Emily enjoys working one on one with the residents, setting up weekly activity programs and incorporating resident's recipes into her menu.
